HARMONY EDUCATION FIELD TRIP LIABILITY PERMISSION SLIP

LIABILITY RELEASE AGREEMENT

I, the undersigned, hereby give my permission for my child/children listed below to participate in any field trips during the 2024-2025 school year. I acknowledge that as a parent/guardian I take responsibility for the supervision of my own children at every field trip. I acknowledge that while the staff of Harmony Education will take every precaution to ensure the safety and well-being of the students, I understand that field trips may involve certain risks, including but not limited to accidents, injury during activities, and unforeseen medical emergencies. I hereby release, discharge, and hold harmless Harmony Education Services, its employees, volunteers, and agents from any liability, claims, or demands arising from my child’s participation in the field trip.

I confirm that I have carefully read this CONSENT AND RELEASE and agree to its terms knowingly and voluntarily. I also confirm that I am the parent or legal guardian of the child/children named.
